A Lenoir day care worker is charged in the death of a 16-month-old child who died at the day care on Monday.
Alexandra Lee Livingston Coffey, 29, of Lenoir, is charged with involuntary manslaughter, the Lenoir Police Department said in a news release.
Lenoir police did not name the child who died. According to started by Megan Cragle, the child’s name is Maddy. Maddy’s mother, Angel Dawn, has made a series of Facebook posts regarding Maddy’s death.

“On May 19, 2025, she sent her two youngest babies to child care for the first time,†the GoFundMe page said. “It was a heartbreaking, nerve wracking day sending her babies off to complete strangers. They have always been in Angel’s care. She received updates on them that they were doing just fine. Until Angel got the call no mother ever wants to get. Her precious baby girl never woke up from nap time.â€

Lenoir police officers, along with the Lenoir Fire Department and Caldwell County EMS, responded to a report of an infant in cardiac arrest at Creative Beginnings of Lenoir at about 2:45 p.m. on Monday. Creative Beginnings is located at 1966 Âé¶¹´«Ã½ Blvd., Lenoir police said in a news release.
Lifesaving measures, including CPR, were done when medics arrived on the scene. The child was taken to UNC Health Caldwell, where resuscitation efforts continued. Maddy died shortly after arriving at the hospital, Lenoir police said.
After receiving preliminary findings from the North Carolina Office of the Chief Medical Examiner and in consultation with the Caldwell County District Attorney’s Office, Lenoir police charged Coffey. Lenoir police did not say how Maddy died or why Coffey was charged.
Coffey is currently being held at the Caldwell County Detention Center under a $500,000 secured bond.
The investigation remains active. Lenoir police said no additional details will be released at this time to protect the integrity of the case.
“The Lenoir Police Department remains committed to a thorough and impartial investigation and continues to work closely with the North Carolina Office of the Chief Medical Examiner and the Caldwell County District Attorney’s Office,†the release said. “We extend our continued condolences to the family of the child and all those impacted by this heartbreaking incident.â€
Coffey is innocent until proven guilty in a court of law.Â
